You may be hearing about Home &amp; Community Care, a referral, equipment, a Case Manager, or services that are expected after you get home.<\/p>\n    <p class=\"kyr-lead\"><strong>You do not need to understand the whole healthcare system.<\/strong><\/p>\n    <p>What matters is knowing:<\/p>\n    <ul>\n      <li>what has actually been arranged;<\/li>\n      <li>what is still pending;<\/li>\n      <li>when the first expected service or visit is;<\/li>\n      <li>who is responsible for arranging important pieces; and<\/li>\n      <li>who you should contact if something is unclear or does not happen as expected.<\/li>\n    <\/ul>\n    <p>This page will help you know what to ask before you leave hospital.<\/p>\n    <div class=\"kyr-main-idea\"><strong>The main idea:<\/strong> Something may have been discussed or requested without yet being confirmed. You do not need to figure out everyone's job.<\/p>\n    <p>Instead, start with a few practical questions:<\/p>\n    <div class=\"kyr-ask\">What has actually been arranged for when I get home?<\/div>\n    <div class=\"kyr-ask\">Is anything still waiting to be confirmed?<\/div>\n    <div class=\"kyr-ask\">When is it expected to happen?<\/div>\n    <div class=\"kyr-ask\">Who is responsible for arranging it?<\/div>\n    <div class=\"kyr-ask\">Who should I contact if I have questions?<\/div>\n    <div class=\"kyr-card kyr-soft-card\">\n      <span class=\"kyr-note-label\">KYR practical preparation<\/span>\n      <p>If something does not make sense, ask for it to be explained again.<\/p>\n      <p>You can also involve a trusted family member, caregiver or support person if that would help you understand and remember the plan.<\/p>\n      <p><strong>These are practical preparation suggestions from KYR. They are not mandatory Alberta discharge requirements.<\/strong><\/p>\n    <\/div>\n  <\/section>\n\n  <section id=\"coordinating\" class=\"kyr-section\">\n    <span class=\"kyr-number\">2<\/span>\n    <h2>Who Is Coordinating the Transition?<\/h2>\n    <p>Depending on your situation, different people may be involved in planning your transition home.<\/p>\n    <p>This could include members of your hospital healthcare team, a discharge or transition professional, Home &amp; Community Care, or a Case Manager where applicable.<\/p>\n    <p>There is not one person who necessarily handles every part of every transition.<\/p>\n    <p>So rather than trying to understand the whole system, ask:<\/p>\n    <div class=\"kyr-ask\">\u201cWho is coordinating or helping with the plan for me to come home?\u201d<\/div>\n    <p>If Home &amp; Community Care will be involved, also ask:<\/p>\n    <div class=\"kyr-ask\">\u201cWho is my Home &amp; Community Care contact?\u201d<\/div>\n    <p>You can also ask:<\/p>\n    <div class=\"kyr-ask\">\u201cHow is the handoff from the hospital to Home &amp; Community Care being made?\u201d<\/div>\n    <p>Write down the name or contact information you are given if that will help you later.<\/p>\n  <\/section>\n\n  <section id=\"arranged\" class=\"kyr-section\">\n    <span class=\"kyr-number\">3<\/span>\n    <h2>What Home &amp; Community Care Has Actually Been Arranged?<\/h2>\n    <p><strong>This is important.<\/strong><\/p>\n    <p>Something may have been discussed, requested or referred without yet being confirmed.<\/p>\n    <p>You do not need to learn special system terminology. You simply need to find out what you should actually expect.<\/p>\n    <p>Try asking:<\/p>\n    <div class=\"kyr-ask\">\u201cWhat Home &amp; Community Care has actually been arranged?\u201d<\/div>\n    <div class=\"kyr-ask\">\u201cHas the referral or handoff happened?\u201d<\/div>\n    <div class=\"kyr-ask\">\u201cIs anything still pending?\u201d<\/div>\n    <div class=\"kyr-ask\">\u201cWho is our contact?\u201d<\/div>\n    <div class=\"kyr-ask\">\u201cIs there anything we are expected to do?\u201d<\/div>\n    <div class=\"kyr-card kyr-warm-card\">\n      <p>A referral or request does not by itself guarantee that a particular service, amount of funding or number of care hours will be provided.<\/p>\n    <\/div>\n    <p>The goal here is to understand your own confirmed transition plan as clearly as possible.<\/p>\n  <\/section>\n\n  <section id=\"first-service\" class=\"kyr-section\">\n    <span class=\"kyr-number\">4<\/span>\n    <h2>When Is the First Expected Visit or Service?<\/h2>\n    <p>KYR did not identify one universal timeframe that applies to every person for when Home &amp; Community Care begins after leaving hospital. Your expected timing depends on your own transition plan.<\/p>\n    <p>That makes this a particularly important question:<\/p>\n    <div class=\"kyr-ask\">\u201cWhen is my first expected Home &amp; Community Care service or visit?\u201d<\/div>\n    <p>If you are unsure, also ask:<\/p>\n    <div class=\"kyr-ask\">\u201cWhat should we do, and who should we contact, if that timing changes or is unclear?\u201d<\/div>\n    <p><strong>Do not assume someone will arrive as soon as you get home unless that is what your own transition plan says.<\/strong><\/p>\n    <p>Knowing the expected timing can help you and your family understand what is supposed to happen next.<\/p>\n  <\/section>\n\n  <section id=\"between\" class=\"kyr-section\">\n    <span class=\"kyr-number\">5<\/span>\n    <h2>What Happens Between Arriving Home and the First Service?<\/h2>\n    <p>There may be time between arriving home and the first expected Home &amp; Community Care visit or service.<\/p>\n    <p>Before you leave hospital, ask what the plan is for that period.<\/p>\n    <div class=\"kyr-ask\">\u201cWhat is supposed to happen between getting home and the first Home &amp; Community Care service?\u201d<\/div>\n    <div class=\"kyr-ask\">\u201cIs there anything we are expected to do?\u201d<\/div>\n    <div class=\"kyr-ask\">\u201cWho do we contact if we do not understand the plan?\u201d<\/div>\n    <div class=\"kyr-ask\">\u201cWho do we contact if the expected service does not happen?\u201d<\/div>\n    <div class=\"kyr-card kyr-warm-card\">\n      <span class=\"kyr-note-label\">Important<\/span>\n      <p>KYR does not provide clinical discharge instructions.<\/p>\n      <p>This page is not telling you or your family how to perform nursing care, medical procedures or other professional care at home.<\/p>\n      <p>If you do not understand clinical instructions you have been given, ask the appropriate member of your healthcare team to explain them.<\/p>\n      <p><strong>Do not assume that a family member is automatically responsible for providing professional care while you wait for an expected service.<\/strong><\/p>\n    <\/div>\n  <\/section>\n\n  <section id=\"responsibility\" class=\"kyr-section\">\n    <span class=\"kyr-number\">6<\/span>\n    <h2>Who Is Responsible for Arranging Each Piece?<\/h2>\n    <p>During a hospital-to-home transition, different people or programs may be responsible for different parts of the plan.<\/p>\n    <p>That means statements such as:<\/p>\n    <div class=\"kyr-ask\">\u201cThe hospital will arrange it.\u201d<\/div>\n    <p>or:<\/p>\n    <div class=\"kyr-ask\">\u201cHome Care will take care of it.\u201d<\/div>\n    <p>may be too broad.<\/p>\n    <p>Instead, for anything important, ask two questions:<\/p>\n    <div class=\"kyr-ask\">\u201cWho is responsible for arranging this?\u201d<\/div>\n    <div class=\"kyr-ask\">\u201cWho should I contact if it does not happen?\u201d<\/div>\n    <p>You can use these questions for services, referrals, equipment or another important part of your Home &amp; Community Care transition.<\/p>\n    <p>You do not need to know the whole organizational structure.<\/p>\n    <p><strong>You need to know who is responsible for the next step in your situation.<\/strong><\/p>\n  <\/section>\n\n  <section id=\"equipment\" class=\"kyr-section\">\n    <span class=\"kyr-number\">7<\/span>\n    <h2>Equipment and Preparing the Home<\/h2>\n    <p><strong>This section applies only if equipment is part of your transition.<\/strong><\/p>\n    <p>Not everyone coming home from hospital needs equipment.<\/p>\n    <p>If equipment is expected, try to understand what has actually been arranged before you leave.<\/p>\n    <div class=\"kyr-ask\">\u201cWhat equipment is expected to be needed at home?\u201d<\/div>\n    <div class=\"kyr-ask\">\u201cHas it been assessed or arranged?\u201d<\/div>\n    <div class=\"kyr-ask\">\u201cIs anything still pending?\u201d<\/div>\n    <div class=\"kyr-ask\">\u201cWho is responsible for arranging it?\u201d<\/div>\n    <div class=\"kyr-ask\">\u201cWhen is it expected?\u201d<\/div>\n    <div class=\"kyr-ask\">\u201cWho should we contact if it has not arrived?\u201d<\/div>\n    <div class=\"kyr-card kyr-soft-card\">\n      <h3>What about AADL?<\/h3>\n      <p>The Alberta Aids to Daily Living (AADL) program may be relevant for some equipment and supplies.<\/p>\n      <p>AADL has its own assessment, authorization and eligibility requirements.
